L'actrice Chris McNeil est inquiète au sujet de sa fillette Regan : après que l'on ait entendu des bruits curieux venant de sa chambre, la petite a changé, proférant de constantes insanités. Une force para-normale l'habite, qui coûte la vie au metteur en scène de Chris. Désespérée, cette dernière fait appel à deux exorcistes... 

« D’où vient la force manifeste de ce film ? De son économie d’effets qui ménage de longues plages de suspens coupant subitement l’aspect monotone des situations par un effet choc. Non pas du gore, non pas des éviscérations en gros plan, mais un dosage subtil et équilibré destiné à tenir en haleine - on ne sait jamais ce qui va se passer - et une habileté à nous plonger dans une horreur quotidienne, décrite comme tout à fait plausible (évolution psychologique des personnages, allers et retours incessants des prêtres, de l’inspecteur de Police, bruits et râles de Regan se retournant dans son lit, etc.). One of the most profitable horror movies ever made, this tale of an exorcism is based loosely on actual events. When young Regan (Linda Blair) starts acting odd -- levitating, speaking in tongues -- her worried mother (Ellen Burstyn) seeks medical help, only to hit a dead end. A local priest (Jason Miller), however, thinks the girl may be seized by the devil. The priest makes a request to perform an exorcism, and the church sends in an expert (Max von Sydow) to help with the difficult job.

‘’THE EXORCIST is one of the best movies of its type ever made; it not only transcends the genre of terror, horror, and the supernatural, but it transcends such serious, ambitious efforts in the same direction as Roman Polanski’s “Rosemary’s Baby.” Carl Dreyer’s “The Passion of Joan of Arc” is a greater film--but, of course, not nearly so willing to exploit the ways film can manipulate feeling. The film does that with a vengeance. The film is a triumph of special effects. Never for a moment--not when the little girl is possessed by the most disgusting of spirits, not when the bed is banging and the furniture flying and the vomit is welling out--are we less than convinced.
